ALL athletes are required to have a physical exam each year prior to participating on a Pallotti athletic team.
Physicals for the 2010 - 2011 school year MUST be dated after May 1st, 2010. Any physical done prior to that date is not valid for this school year.
Students CANNOT tryout, practice, or play until the physical forms are turned into the Athletic Trainer (NO EXCEPTIONS).
Athletic Healthcare forms may be obtained by:
• Picking them up in the Athletics’ office
• Downloading the forms from our web site by clicking a form link on the right.
There are several ways to obtain an athletics physical.
• See your personal physician
• Attend any one of the several clinic offerings
• Go to Patient First in Laurel (301.497.1820)
